- An authorization letter signed by the original requestor (owner of GMC)
- Photocopy of both the owner's ID and the authorized person's ID cards
- Official Receipt (OR) of payment
REQUEST NOT CLAIMED WITHIN 90 DAYS WILL BE DISCARDED.
